CARE PACKAGES ARE OPEN TO ANYONE IN THE US! Nominate a friend or a loved one with epilepsy! Include YOUR info in the first top three boxes. Put the nominees first & last name, and age in the box below. Then provide their address. Be aware due to our care package inventory constantly changing the nominee may not get one as pictured below- ether way it will be filled with something your nominee will LOVE!

We have a suggested donation of 15 dollars to cover shipping & product expenses. You will be redirected after you fill out the form to our donation page. it is not mandatory, but we are a small non-profit, and we are grateful for any donation!
